The first visit to our Flagstaff dental office is designed to get you better acquainted with all we offer as well as introduce you to Dr. Geisel and our caring staff. We encourage questions and do our best to always deliver quality care.

Please take a moment prior to your scheduled appointment to download our patient forms. We ask that you complete the forms and bring them with you to your appointment so we may better assist you in a timely manner. Thank you for your confidence in our office, we look forward to assisting you with all your dental needs.

If you have been seen within the last five years and have had x-rays taken, it is important that we receive copies of these from you previous dentist.

If they are of good quality, it may not be necessary to have them taken at you first visit here.  Please complete a “records release” form and return it to the office.  If no x-rays have been taken, or if the ones forwarded to us are not of good quality, we will need to take diagnostic films at your first visit in our office.

Some dental insurance plans have limits on their coverage of radiographs.  If you have dental insurance, please check with your policy’s coverage.  Good quality x-rays taken at your previous dentist may be used by our office.  If they are of poor quality, or if we do not receive them from your previous dentist before your first visit we may need to take x-rays.

You may be responsible for the fees involved, as insurance may not cover these!  Please call the office if you have any questions regarding our x-ray policy.
